Funding for health coverage enrollment assistance continues in Southwest Colorado


 

 

The La Plata Family Centers Coalition is pleased to announce it has received continuation funding from the Colorado Health Foundation to conduct health coverage outreach and enrollment activities in Archuleta and La Plata counties. The $293,096 award will extend current funding from the Foundation through July 2017 to increase health literacy and further reduce the number of uninsured individuals and families in Southwest Colorado.

The Foundation funding supports outreach and enrollment activities of the following Medical Coverage Collaborative partners: Axis Health System, Archuleta County Department of Human Services, La Plata County Department of Human Services and the La Plata Family Centers Coalition. All partners provide trained enrollment specialists at their sites to assist individuals and families in obtaining health insurance during open enrollment season, November – January, and throughout the year for those with a qualifying life event or who need assistance with applying for the state’s Medicaid program, Health First Colorado, or Child Health Plan Plus (CHP+).
